To make a New Patient appointment, please email  reception @ wholefamilymd. org, and type "New Patient" in the subject line. You should recieve a response within 1 week. If you do not hear back within that time frame, please call the office to make sure your email was recieved.

 In requesting your appointment, please supply the following information:

• Which doctor you wish to see
• Your name
• Your best contact phone number

 Upon confirming your appointment, we will require a Visa or Mastercard #, your mailing address, and date of birth, which we will collect over the phone. For your safety, please do not include this information in your Appointment Request email.
